29 CFR 1910.110(h)(6)(ii)(b): Propane tanks were not adequately protected by crash rails or guards to prevent physical damage unless they are so protected by virtue of their location: On or about December 3, 2014, and at times prior thereto, at the propane tank storage area the propane tank was not adequately protected from damage caused by forklift trucks when employees are refueling their forklift trucks. Employees were exposed to explosion and fire hazards.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: On or about December 3, 2013, and at times prior thereto, in the tool & room and maintenance shop the employer allowed employees to operate abrasive wheel grinding machines that either did not have the tongue guard provided or the distance between the tongue guard and the wheel exceed one-fourth inch. Employees were exposed to flying fragments in the event of an exploding wheel.

Employees were exposed to amputation hazards.
