29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.  a) At the establishment, the Mori Seiki SL-1A CNC lathe was not guarded in that the lathe did not stop turning when the door was opened exposing the employee to the point of operation and rotating parts resulting in broken bones, amputation, and death.   b) At the establishment, the Mori Seiki SL-3 CNC lathe was not guarded in that the lathe did not stop turning when the door was opened exposing the employee to the point of operation and rotating parts resulting in broken bones, amputation, and death.   c) At the establishment, the Mori Seiki SL-25 CNC lathe was not guarded in that the lathe did not stop turning when the door was opened exposing the employee to the point of operation and rotating parts resulting in broken bones, amputation, and death.   d) At the establishment, the Mazak SQT-15M CNC lathe was not guarded in that the lathe did not stop turning when the door was opened exposing the employee to the point of operation and rotating parts resulting in broken bones, amputation, and death.   e) At the establishment, the Mazak VTC-41 CNC lathe was not guarded in that the lathe did not stop turning when the door was opened exposing employees to the point of operating and rotating parts resulting in broken bones, amputation, and death.

29 CFR 1910.215(a)(2): Guard design. The safety guard shall cover the spindle end, nut, and flange projections. The safety guard shall be mounted so as to maintain proper alignment with the wheel, and the strength of the fastenings shall exceed the strength of the guard:     a) At the establishment, the company used a Rockwell Single Voltage Grinder with buffing wheels to buff finished product. The spindle ends and the nuts holding the buffing wheels on the spindles were not guarded exposing employees to caught-by hazards.

Machines designed for a fixed location shall be securely anchored to prevent walking or moving.  a) At the establishment, the Delta Rockwell drill press had not been anchored to prevent the press from walking or moving exposing employees to struck-by/caught-by hazards.
