29 CFR 1926.453(b)(2)(iv): Employee(s) working in an aerial lift were sitting or climbing on the edge of the basket: (a). Worksite located at 6320 N. Chatham Ave., Kansas City, Missouri- An employee was exposed to a fall hazard in that the employee was observed disconnecting his lanyard from the aerial lift and climbing on the guardrails of the lift in order to access the top of the parapet wall and on to the roof.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards: (a). Worksite located at 6320 N. Chatham Ave., Kansas City, Missouri- Employee working in an aerial lift had not been properly trained on the necessity of standing on the floor of the platform and not disconnecting a lanyard and climbing on the guardrails of the lift.
