29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    a.   On or about December 3, 2013, south east side of the house, employees picking up debris were not wearing hard hats while working below employees on a roof, exposing themselves to the hazard of being struck by falling debris.

29 CFR 1926.252(a): An enclosed chute of wood, or equivalent material, was not used where materials were dropped more than 20 feet to points lying outside the exterior walls of the building(s):  a.   On or about December 3, 2013, south/east side of the house, employees working on the top of the house were tossing debris from 25 feet high to the ground level, exposing employees below to the hazard of being struck by falling debris.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  a.   On or about December 3, 2013, on a 4/12 pitch roof, employees were not protected from a 25 feet and a 23 feet 9 inches fall hazard by the use of a fall protection system.  b. On or about December 10, 2013, on a 4/12 pitch roof, an employee was not protected from a 24 feet fall hazard by use of a fall protection system.
