29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: At the job site, 639 W. 2nd, Hutchinson, Kansas, the employer had not developed a Lockout Tagout energy control program when performing service and maintenance on machines and equipment. Employees were exposed amputation hazards from the unexpected startup and energization of equipment such as the brake presses and air compressor.

29 CFR 1910.215(a)(4): On offhand grinding machines, work rests were not used to support the work and/or were not kept adjusted closely to the wheel with a maximum opening of one-eighth inch to prevent the work from being jammed between the wheel and the rest, which may cause wheel breakage. At the job site, 639 W. 2nd, Hutchinson, Kansas, employees were operating the Dayton grinder in the welding shop without the work rest properly adjusted. Work rest gap between the wheel and work rest was over 1/8 inch. Employees were exposed to caught in hazards.
