29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: 2 Northeast Blvd, Sterling, MA On 8/2/13 and at times prior, the employer had not documented and used energy control procedures for employees who cleaned out the agglomerator section of the regrind machine, exposing them to crushing hazards. On 8/2/13, an employee received a partial finger amputation while on the far side of that machine while another employee pushed the down button and the agglomerator closed.
