On or about December 4, 2013, damaged electrical equipment in the following instances exposed employees to an electrical hazard: a: The extension cord powering the Makita 355 mm Portable Cut-Off Wheel had a damaged outer sheathing exposing the internal wiring. b: Conduit on the outer wall of the batch plant powering the light in the plant bay had separated, exposing the internal wiring.
