29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees:     On or about December 5, 2013, centered inside the framed house under construction, the employer did not ensure that electrical equipment was free from recognized hazards. Employees were exposed to electrical shock hazards in the following instances:         a)    A 100-foot orange extension cord (laid in a puddle of water) used to power the Milwaukee Orbital Sawzall (Model #6520-21/Serial #B020610250067) had its strain relief damaged at the female end and damage to the outer insulation.        b)    A 100-foot orange extension cord (laid in a puddle of water) used to supply power to the hand-made electrical handy-box had its ground pin removed and damage to the outer insulation.        c)     The hand-made electrical handy-box (laid in wet conditions) cord had its ground pin removed and was not properly spliced.

29 CFR 1926.403(b)(1)(i): Safety of equipment was not determined on the basis of suitability of the equipment for an identified purpose as evidenced by listing, labeling, or certification for that identified purpose:    On or about December 5, 2013, inside of the framed house under construction, employees were getting electrical power (to operate their power tools) from a hand-made electrical handy-box with a single two duplex receptacle (15A-125VAC 2-pole 3-wire) that had no cover with bare electrical conductors were exposed. Employees were exposed to electrical shock hazards.

29 CFR 1926.405(j)(3)(i): Appliances, other than those in which the current carrying parts at high temperatures are necessarily exposed, had live parts normally exposed to employee contact:     On or about December 5, 2013, outside of the framed house under construction, the 1750-rpm motor on Quincy Air Compressor (Model #111DV8PN3A, Serial #20060718-0148) had exposed live parts. Employees were exposed to electrical shock hazards.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:    On or about December 5, 2013, outside of the framed house under construction, three employees were working on the top-plate and outer edges of the framed house. The employee(s) were exposed to a fall hazard of greater than six feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access:    On or about December 5, 2013, , the 20-foot Werner Extension Ladder positioned in front of the framed house under construction did not have the ladder side rails extend at least 3 feet (.9 m) above the roof surface. Employees were exposed to fall hazard of approximately 10-feet.
