29 CFR 1910.303(b)(7)(i) Unused openings in boxes, cabinets, equipment cases, and housings were not effectively closed to afford protection substantially equivalent to the wall of the equipment. Employees near the Muller Martini machine were exposed to electrical hazard as a result of unused openings in a 240 volt electrical box. 29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws. Employees on the workroom floor were exposed to hazard of electrical shock because flexible conduit had pulled from its anchorage point and protection was not provided to internal electrical conductors.
