NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR HIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDDITIONAL PENALTY OF $1,000 IN ACCORDANCE WITH 29 CFR 1903.19.
