29 CFR 1910.1030(c)(1)(ii)(B):  The employer's Exposure Control Plan did not include the schedule and method of implementation for paragraphs (d) Methods of Compliance, (f) Hepatitis B Vaccination and Post-Exposure Evaluation and Follow-up, (g) Communication of Hazards to Employees, and (h) Recordkeeping, of this standard.  a) Saint Mary's Home of Erie DBA Saint Mary's East- On or about 12/6/13, the written Bloodborne Pathogen Exposure Control Plan was not site specific in that it did not identify the following:    1.) procedures for the annual evaluation of safety engineered sharps currently in use.  The method of implementation was not described, nor was a start date identified;    2.) the healthcare professional that will conduct the confidential post exposure evaluation and follow up in the event of an exposure incident;     3.) the person responsible for maintaining employee medical records;    4.) a description of, or procedures for, maintaining confidentiality of employee medical records;    5.) a description of how Bloodborne pathogen training will be conducted and who is responsible for conducting the training.

29 CFR 1910.1030(c)(1)(iv)(B): The employer's exposure control plan did not document annually the consideration and implementation of appropriate commercially available and effective safer medical devices designed to eliminate or minimize occupational exposure:  a) Saint Mary's Home of Erie DBA Saint Mary's East- On or about 12/6/13, the employer had not documented the annual consideration of safer medical devices for devices currently in use at the facility.  Employees had potential exposure to bloodborne pathogens intermittently throughout each day, when for instance, giving injections, initiating IVs, treating wounds, or cleaning blood or other potentially infectious material (OPIM) spills.

Saint Mary's Home of Erie DBA Saint Mary's East- On or about March 11, 2014, and times prior thereto:  Housekeeping employees worked with or around corrosive chemicals including, but not limited to, Sodium Hypochlorite and Phosphoric Acid, which were contained in "Sani-Shield" and "Commercial Solutions Ultra Clorox Germicidal Bleach I".  The housekeeping employees were not provided with, and did not have access to, adequate eyewash facilities.
