29 CFR 1910.95(g)(1): The employer did not establish and maintain an audiometric testing program by making audiometric testing available to all employees whose exposures equal or exceed an 8-hour time-weighted average of 85 decibels: (a) Millet Plastics, Inc. - The employer did not establish an audiometric testing program for a machine operator who was exposed to noise of 63.2% of the permissible daily noise exposure (85dBA) during 431 minutes of personal sampling. The employee's average noise exposure was approximately 87.5dBA, on or about December 19, 2013. (b) Millet Plastics, Inc. - The employer did not establish an audiometric testing program for a machine operator who was exposed to noise of 94.3% of the permissible daily noise exposure (85dBA) during 438 minutes of personal sampling. The employee's average noise exposure was approximately 90.2dBA, on or about December 19, 2013. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by moveable parts on the blow molding machine: (a) Millet Plastics, Inc. - Employees deactivate the interlocked safety system of the protective guards on the Uniloy Milacron blow molding machine prior to entering the work area. Employees enter the area to retrieve plastic parts that had fallen from the blow mold mechanism, on or about December 6, 2013. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
