29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift: On or about December 6, 2013, employee operating an aerial lift was not wearing any type of fall protection. Employee was changing the electrical ballast on the utility pole light. This condition exposed employee to falls and struck-by hazards exceeding 4 feet.
