Vantage Healthcare Linen Services, LLP, 2300 Colonial Avenue, Erie, PA 16506, clean side, on or about December 6, 2013 and times prior thereto: The outer jackets of the cords for the pendant boxes were frayed. The cord's outer jackets were pulled away from the pendant box and exposed the inside insulated conductors.

Vantage Healthcare Linen Services, LLP, 2300 Colonial Avenue, Erie, PA 16506, on or about April 2, 2014 and times prior thereto: All employees, except for maintenance staff, are to evacuate the building in the event of a fire alarm / fire emergency. Maintenance staff was trained and permitted to use fire extinguishers. The emergency action plan did not identify the maintenance staff and their role in the event of a fire alarm / fire emergency.
