29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On December 3, 2013 and at times prior thereto; North side of the residential home; the ladder used to access the upper floor platforms were extended flush to the platform/landing exposing them to a fall hazard of 15 feet to ground level.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) On December 3, 2013 and at times prior thereto; top floor area; employees were observed working near the edge of the platform without a personal fall arrest system in place to prevent them from falling 30 feet to ground level. a) On December 3, 2013 and at times prior thereto; second floor; employees working near unguarded wall openings without a personal fall arrest system in place to protect them from falling 12-15 feet to ground level. The A/X Carpenters LLc was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.501(b)(13), which was contained in OSHA inspection number 316182872, citation number 1 item number 3 and was affirmed as a final order on January 1, 2012, with respect to a workplace located at 5420 Galena Pl NW Washington DC.
