29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: (a) Jobsite, 3431 Butler Street, 4th Level, Pittsburgh, PA: On or about 12/05/13 an employee was working at various unprotected edges approximately 9 1/2 - 40 feet and no fall protection systems such as guardrails, safety nets or personal fall arrest were in use. The employee was assisting another employee in the installation of microlams and depending on his position had a fall hazard of approximately 9 1/2 feet to the floor below or approximately 20 feet to the "bump out" area.

[59 FR 40738, Aug. 9, 1994; 60 FR 5131, Jan. 26, 1995] (a) Jobsite, 3431 Butler Street, Pittsburgh, PA: On or about 12/05/13 the employer did not provide retraining for employees who were working on the 4th level of the structure where fall hazards existed indicating that the employees had not retained the requisite knowledge and understanding of fall protection systems and the proper use of such equipment to protect them from falls.
