29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:   a) 3rd floor, Shafts # 3/4 :  Inadequate cable protection was installed on the front area on the shaft, and no protection was provided on the rear area of the shaft. Employee(s) were exposed to a seven (7) foot fall into the shaft, on or about 11/22/2013.

Employees were exposed to trip hazards; on or about 11/22/2013.
