OSHA Act of 1970 Sections 5(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing for likely to cause death or serious physical harm to employees,  in that employees were exposed to crush and struck by hazards while operating industrial trucks:     (a) Dock#8  On or about December 7, 2013, the employer did not ensure that trailers could not be moved from the dock area while employees operating powered industrial trucks were still inside loading/unloading.    Among other means, one feasible and acceptable abatement method to correct this hazard is to follow the manufacturers requirements outlined in the Owners Manual for the TL85A vehicle restraint with Blue Genius Controls by conducting routine inspections of the TL85A vehicle restraint with Blue Genius Controls, removing from service any malfunctioning dock lock system until it has been repaired, and train employees in the operation of the TL85A vehicle restraint with Blue Genius Control.
