A.) Gilcon & Associates, Inc.; Poplar Street construction site, Meadville, PA - On or about 12/9/13, employees were working on the roof of a structure that was 17 feet above ground level. The employees were not utilizing any type of fall protection system while on the roof.

29 CFR 1926.1053(b)(8): Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder. A.) Gilcon & Associates, Inc.; Poplar Street construction site, Meadville, PA - On or about 12/9/13, employees were working on the roof of a structure that was 17 feet above ground level. The employees were accessing the area with a 25 foot extension ladder that was not secured to prevent accidental displacement.
