OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards: A) Combo platform, on or about December 5, 2013, the ladder access to the combo platform did not have a gate or chains across the ladder opening and there were open sided floors on the platform by the ladder, exposing employees to a fall hazard of approximately 18 feet. B) Platform below combo platform, on or about December 5, 2013, the ladder access to the platform did not have a gate or chains across the ladder opening and there were no guardrails on the open sided floor area, exposing employees to a fall hazard of approximately 8 feet. C) Mote separate platform, on or about December 5, 2013, the ladder access to the Mote separate platform did not have a gate or chains across the ladder opening on to the platform exposing employees to a 10 feet fall hazard. D) Behind cotton press baler machine, on or about December 5, 2013, the stairs leading to the pit area did not have stair-rails in place at the top when the floor grating was removed to allow access to the stairway and there was no mid-rail as part of the stair- rail system, exposing employees to a fall hazard of approximately 15 feet. E) Outside of office by Cotton press, on or about December 5, 2013, the stairs leading to the up-stairs parts room did not have a mid-rail in place and the portable metal stairs used to access the work level area did not have mid-rails, exposing employees to fall hazards between.
