OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that the employer did not ensure a like for like replacement was made when changing equipment on an ammonia refrigeration system. a) In the machine room, on or about 12/7/13: When changing gaskets on a compressor for an ammonia refrigeration system, the employer did not ensure the new gasket was a like for like replacement to the old gasket which had been in use. When starting up the compressor, the gasket popped and anhydrous ammonia was released into the building. Abatement certification must be submitted for this item.
