29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition.  A) Pellet Mill, on or about October 17, 2013, the employer did not ensure that all elevated surfaces were kept free of hazardous accumulations of combustible dust greater than 1/32 of an inch, exposing employees to a fire, deflagration, and dust explosion hazard.    B) Storage building, on or about December 17, 2013, electrical motors and electrical boxes were covered with combustible dust greater than 1/32 of an inch, and some electrical boxes had dust inside them, exposing employees to a fire, deflagration, and dust explosion hazard.

Exposing employees to rotating parts hazards and in-running nip point hazards.   B) Cooler Aspirator belt conveyor, on or about December 17, 2013, the rotating coupling for the conveyor belt drive motor was not covered or guarded, exposing employees to a rotating part hazard.
