On or about 1/28/14, Utica Cutlery Company did not record the following cases on the 2014 OSHA 300 Log: a) A recordable injury occurred on 1/6/14 when an employee is still out of work due to right hand skin loss and loss of a fingernail. b) A recordable injury occurred on 1/6/14 when an employee had medical treatment, sutures from a lacerated left finger. c) A recordable injury occurred on 1/17/14 when an employee had lost work days from 1/22/14 - 2/27/14 due to a shoulder strain.
