29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift. On or about December 4, 2013, at 1050 Handley Dr., Fort Worth, Texas: an employee worked from a vehicle-mounted rotating work platform without wearing a personal body harness and lanyard or body belt with a lanyard attached to the boom or basket to prevent a fall to the ground of approximately 12 feet.
