Abatement Certification and Documentation is required: On or about December 23, 2013, the employer did not maintain the OSHA 300 Log and Summary of Occupational Injuries and Illnesses. An employee sustained crushed fingers on November 25, 2013, which resulted in medical treatment other than first aid.
