29 CFR 1910.151(c) Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use.  In the kitchen for employees who used corrosive chemicals including but not limited to Greasestrip Plus oven degreaser and Lime-A-Way, an approved eyewash was not available for immediate emergency use.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  Cooks use a Hobart Mixer, m/n A200, s/n 11-149-583, to mix food ingredients which did not have a guard to protect the rotating agitator shaft.

29 CFR 1910.215(a)(4): On offhand grinding machines, work rests were not used to support the work and/or were not kept adjusted closely to the wheel with a maximum opening of one-eighth inch to prevent the work from being jammed between the wheel and the rest, which may cause wheel breakage.   Location:  Maintenance Area  The right side of the Westward 8-inch Bench Grinder used by maintenance workers was missing the work rest.

Distances shall be measured from the live parts if they are exposed or from the enclosure front or opening if they are enclosed:  a) Main Service Area, disconnect for main feed , labeled 58855 1 of 3, 2000 Amp, had a wheel chair and a carpet cleaner stored directly in front blocking access  b) Main Service Area, Sub panel LPB had a cardboard box of rags and a cardboard box of aluminum cans stored in front of it  c) Division 300 soiled utility room, the sub panel LP1 was blocked by a soiled linen cart  d) Division 300 oxygen storage room, subpanel LP3 had oxygen cylinders stored in front of the panel
