29 CFR 1910.147(c)(6)(i): The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed:  Plant and Machine Shop:  Required periodic inspections of lockout procedures for equipment such as but not limited to, lathes and milling machines, were not conducted in over 16 months.

29 CFR 1910.212(a): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:   Shipping & Receiving area: Cutting edges of the blade of a vertical metal band saw were not guarded to prevent operator injuries.      Maintenance Shop: Exposed unused run of the blade of a metal band saw above the roll guides was not guarded to prevent operator injuries.   Chuck and drill bit of a drill press were not guarded to prevent employee injuries.

29 CFR 1910.215(a)(4): On offhand grinding machines, work rests were not used to support the work and/or were not kept adjusted closely to the wheel with a maximum opening of one-eighth inch to prevent the work from being jammed between the wheel and the rest, which may cause wheel breakage:  Center Milling area: Employees were exposed to hazardous conditions where a work rest was missing from a Dayton 6 inch pedestal abrasive grinder.
