The safety guard shall be mounted so as to maintain proper alignment with the wheel, and the strength of the fastenings shall exceed the strength of the guard:    East, clean-up department, Baldor pedestal grinder: on December 10, 2013, employees grinding off welds on steel parts were exposed to laceration and shrapnel struck-by hazards from shattering of abrasive wheels where required adequate side wheel guards were not provided. 29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.    (a) East, clean-up department, Amrox BS925 vertical band saw: on or about December 10, 2013, employees cutting variety of metal stock such as, but not limited to, battery supply brackets, were exposed to amputation hazards where unused portion of the running blade was not guarded.    There is no abatement certification or documentation required for this item (a).    (b) Center, press brake department, Cincinnati 51622 and Wysong MTH100-120 hydraulic press brakes: on or about December 10, 2013, employees bending and forming variety of sheet metal stock were exposed to amputation hazards where adequate point of operation guarding systems or methods were not provided.    - Cincinnati 60 CBII x 4ft. serial no. 51622, "small" hydraulic press brake.  - Wysong MT7-141, serial no. MTH100-120, "big" hydraulic press brake.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).    The J. P. Bell Fabricating, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(3)(ii), which was contained in OSHA inspection number 314035387, citation number 01, item number 004 and was affirmed as a final order on 6/21/2010, with respect to a workplace located at 617 N. Central Ave., Wood Dale, IL 60191.
