29 CFR 1910.22(b)(1): Where mechanical handling equipment was used, sufficient safe clearances were not allowed for aisles, at loading docks, through doorways and wherever turns or passage had to be made:    Location: Yard, rock crushing plant vibrating pan and primary jaw    On or about 11/26/13, the employer did not maintain the passage way to the vibrating pan and primary jaw platform, due to dirt and rocks that were being fed by the excavator onto the vibrating pan and primary jaw, exposing employees that access and work on the platforms to slip and trip hazards.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    Location: Yard, vibrating pan and primary jaw       On or about 11/26/13, the employer did not utilize energy control procedures to protect employees from the unexpected start up and amputation hazards while performing un-jamming of the primary jaw to the rock crusher.     Deloury Industries dba Agretech, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1910.147(c)(4)(i), which was contained in OSHA inspection number 283056, citation number 1, item number 1, and was affirmed as a final order on July 5, 2012, with respect to a workplace located at 50 Jackson Street, Dracut, MA.
