29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): Location: Lathe Department On or about 12/5/13, machine operators opened the by-passed and/or damaged interlocking door guard while the spindle was spinning to observe the alignment of the parts to the spindle, exposing the operators to point of operation and amputation hazards. The employer by-passed the interlocking door guarding device on the following but not limited to CNC machines; 1. Hardinge Cobra 42 CNC machine by-passed interlocking door guard with a paperclip jammed in the interlock. 2. Supermax, model YCM-TC-2 CNC machine no interlocking door.
