29 CFR 1910.95(g)(1):  The employer did not establish and maintain an audiometric testing program as provided in this paragraph by making audiometric testing available to all employees whose exposures equal or exceed an 8-hour time-weighted average of 85 decibels.       (a)  Middle Welding Bay:  An employee conducting welding and grinding on fabricated metal was exposed to occupational noise at 79.38% of the permissible daily noise exposure (eight hour time weighted average of 90 dBA at the 80 dB criterion level) during the 456 minute sampling period on December 18, 2013.  For a full eight hour shift this would be equivalent to an average sound level of 88.3 dBA.  The employer has not instituted and maintained an audiometric testing program for all employees exposed at or above 8 hour time weighted average of 85 decibels, as determined on December 18, 2013.        (b)  End Welding Bay:  An employee conducting welding and grinding on fabricated metal was exposed to occupational noise at 166.5% of the permissible daily noise exposure (eight hour time weighted average of 90 dBA at the 80 dB criterion level) during the 455 minute sampling period on December 18, 2013.  For a full eight hour shift this would be equivalent to an average sound level of 93.6 dBA.  The employer has not instituted and maintained an audiometric testing program for all employees exposed at or above 8 hour time weighted average of 85 decibels, as determined on December 18, 2013       Abatement Certification Required for this Item.

29 CFR 1910.95(k)(1): The employer did not train each employee who is exposed to noise at or above an 8-hour time-weighted average of 85 decibels in accordance with the requirements of 29 CFR 1910.95(k). The employer did not institute a training program and ensure employee participation in the program:     (a)  Middle Welding Bay:  An employee conducting welding and grinding on fabricated metal was exposed to occupational noise at 79.38% of the permissible daily noise exposure (eight hour time weighted average of 90 dBA at the 80 dB criterion level) during the 456 minute sampling period on December 18, 2013. The purpose of audiometric testing, and an explanation of the test procedures.     The training program shall be conducted annually for each employee and shall be updated to be consistent with changes in protective equipment and work processes.     Abatement Certification Required for this Item.

29 CFR 1910.303(b)(1): Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees:     (a)  Boring Machine Area:  Employees operate an Konecrane 2 ton overhead crane using a pendant control.  The crane pendant was missing an button leaving a one inch diameter opening in the pendant exposing employees to 120 volt insulated energized conductors, as observed on December 11, 2013. 29 CFR 1910.212(a)(1):  One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:     (a)  Machine Shop:  Employees operate a Kingston metal working lathe that was not equipped with machine guarding for the rotating 12 inch chuck, as determined on December 11, 2013.          The Wilson Welding Company, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 313377053, citation number 1, item number 1 and was affirmed as a final order on November 10, 2010, with respect to a workplace located at 221 High Street - Altizer Addition, Huntington, WV  25705.
