29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: The employer failed to install and use equipment in accordance with the instructions listed and permitted uses. A knockout type box was installed as a pendant and used in the production area near the MBO and Baum Folding machines. Machine components and attachments were plugged in to these boxes. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met: Employees were exposed to chemicals such as Drier Stimulator, Plate Cleaners, A-60 Odorless, Wash, VSH Inks when conducting production operations and during cleaning. The employer did not develop, implement and maintain a site-specific written hazard communication program. Abatement documentation and certification is not required for this item.
