On or about December 10, 2013, four employees performing plastering work were observed using a frame scaffold platforms with heights greater than 10 feet from the ground that was not provided with guardrail system installed on all open sides and ends. These conditions exposed employees to fall hazards.

29 CFR 1926.452(c)(2): Frames and panels were not braced by cross, horizontal, or diagonal braces, or combination thereof to secure vertical members together laterally, square and align vertical members, so that the erected scaffold is always plumb, level, and square. On or about December 10, 2013, four employees performing plastering work were using frame scaffold elevated platforms, ranging from 6 to 10 feet 8 inches in height from the ground. The frames and panels were not braced by cross, horizontal or combination thereof to secure vertical members from swaying laterally, maintain square and aligned vertical members. This condition exposed employees to fall hazards.

29 CFR 1926.451(h)(1): Each employee on a scaffold was not provided with additional protection from falling hand tools, debris, and other small objects through the installation of toeboards, screens, guardrail systems, or through the erection of debris nets, catch platforms, or canopy structures that contain or deflect the falling objects. On or about December 10, 2013, employees performing plastering work were not protected against falling objects from erected scaffold working surfaces above 10 ft. This condition exposed employees to struck-by hazards from falling objects.
