Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: On or about December 11, 2013, employees were exposed to a fall of approximately 24 feet to the ground level below, while removing and replacing shingles on the roof, with an approximate roof pitch 6:12.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access: On or about December 11, 2013, employees were exposed to a fall of approximately 24 feet to the ground level below, as the rails of the extension ladder extended approximately 18 inches above the roof, where they gained access to the roof to replace shingles.
