29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: a) The employee and foreman entered and exited the structure through the main entrance without head protection while the sub-contractor's employee was overhead setting roof trusses, exposed to struck-by hazards, on or about 12/11/13.
