29 CFR 1910.176(a): Where mechanical handling equipment was used, sufficient safe clearances were not allowed for aisles, at loading docks, through doorways or wherever turns or passage had to be made:  (a) Racking V162 - Employees operate Crown stand- up type material handler with an open operator's compartment measuring approximately 47 to 92 inches high while the horizontal racking system bar measured approximately 84 inches above the floor.  The employer did not insure that sufficient safe clearance was provided to prevent employees from being pinned between the material handler and the horizontal racking system bar, on or about December 11, 2013.   (b)  Racking X32 - Employees operate Crown stand- up type material handler with an open operator's compartment measuring approximately 47 to 92 inches high while the horizontal racking system bar measured approximately 72 inches above the floor.  The employer did not insure that sufficient safe clearance was provided to prevent employees from being pinned between the material handler and the horizontal racking system bar, on or about December 19, 2013.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
