29 CFR 1910.29(a)(3)(v): Work platforms were not secured in place: On or about December 11, 2013, in the shop area, the employer did not ensure the Louisville portable platforms were safe to climb. Employee(s) exposed to a fall hazard. Both platforms had the rubber mounts/grips missing or worn through and they would not lock in place. The model number on the Louisville platforms were both SW2425.
