29 CFR 1910.146(c)(4): When the employer decided that its employees would enter permit spaces, the employer did not develop and implement a written permit space entry program that complied with 29 CFR 1910.146: On or about December 10, 2013, and at times prior thereto, at the establishment employees were required to enter permit required confined spaces but the employer did not develop and implement a written permit space entry program that complied with CFR 1910.146. Employees were exposed to inhalation and trip and fall hazards that could result from the blasting operation.

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour: On or about December 10, 2013, and at times prior thereto, at the compressed gas storage area near the Antec building, oxygen and acetylene cylinders were found stored together without a noncombustible barrier. Employees were exposed to the hazard of fire.

29 CFR 1910.254(d)(9)(iii): Joining lengths of work and electrode cables were not done by the use of connecting means specifically intended for the purpose: On or about December 10, 2013, and at times prior thereto, in the Dragon building two welding ground cables were spliced by just using electrical tapes wrapped around the cables and bare conductors were evident. Employees were exposed to electrical shock.
