29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: a) 3601 Spruce St, Philadelphia, PA The employer did not instruct each employee in the recognition and avoidance of unsafe conditions, in that the employees were not trained in the correct operation and rigging of the omega rail lifter.This violation exposed the employees to a struck-by hazard. This violation was observed on or about September 27, 2013. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: a) 3601 Spruce St, Philadelphia, PA The employees were operating an energized (110 volts) electrical MY-TE winch hoist which had a damage strain relief, thus exposing the employees to an electrical shock hazard. This violation was observed on or about September 27, 2013. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
