29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees:  Instances including but not limited to the following:  a) Jobsite, 1734 Penn Avenue, Pittsburgh, PA: On or about January 29, 2014, during cleaning/sanitizing activities on the Daniels Food Company grinder, model number DG 1056, located on the main production floor, the employer did not ensure that appropriate energy control training was provided to the staff in that the lockout training provided by the employer taught employees to perform this work under the supervisors' lock and did not teach them to lockout the equipment themselves.  b) Jobsite 1734 Penn Avenue, Pittsburgh, PA: On or about January 29, 2014, during cleaning/sanitizing activities on the Reiser stuffer machine, serial number 128.2464, located on the main production floor, the employer did not ensure that appropriate energy control training was provided to the staff in that lockout training provided by the employer taught employees to perform this work under the supervisors' lock and did not teach them to lockout the equipment themselves.

29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees:  Instances including to the following:  a) Jobsite, 1734 Penn Avenue, Pittsburgh, PA: On or about January 29, 2014, during cleaning/sanitizing activities on the Daniels Food Company grinder, model number DG 1056, located on the main production floor, the employer did not ensure that employees performing the work placed locks on the energy isolating devices prior to the start of this work in that they were permitted to work under their supervisors' lock.   b) Jobsite, 1734 Penn Avenue, Pittsburgh, PA: On or about January 29, 2014, during cleaning/sanitizing activiities on the Resier stuffer machine, serial number 128.2464, lcoated on the main production floor, the employer did not ensure that employees performing the work placed locks on the energy isolating devices prior to the start of this work in that they were permitted to work under their supervisors' lock.

29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  a) Jobsite, 1734 Penn Avenue, Pittsburgh, PA: On or about January 14, 2014, during cleaning/sanitizing activities on machinery on the main production floor, the employer did not ensure they eye/face protection was used while mixing cleaning chemicals such as KP 3510, a chemical listed as a corrsoive on the material safety data sheet.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and maintain at each workplace, a written hazard communication program which at least described how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, safety data sheets, and employee information and training will be met, and which also includes the following:   a) Jobsite, 1734 Penn Avenue, Pittsburgh, PA: On or about January 14, 2014, the employer did not ensure that a written hazard communication program was in place at the site. Employees used chemicals such as KP 3510 and bleach, to clean/sanitize with at the site.
