Employees were exposed to electrocution, electrical shock and fire hazards in areas such as but not limited to:    a) near the Hufford stretch press on the cable tray from the ceiling flexible cord ran down and connected into two metal receptacle gang boxes and flexible cord ran out of metal gang boxes into a metal on/off switch box;  b) at the 805 line, the Kalamazoo horizontal saw 480 3 phase switch box had a flexible cord routed into the top of the box and mounted approximately 4 feet off the floor; and  c) at the JIC fabrication line on the ACC cable tray, flexible cords were routed through four metal gang boxes hanging from the cable tray.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:    On or about December 11, 2013 and at time prior thereto, the employer did not ensure that strain relief devices were installed to prevent tension, strain, pull or abrasion of the electrical conductors and cords, exposing employees to the hazards of electrocution, electrical shock and fire in areas such as but not limited to:    a) in the 520 and 570 grind tables and weld areas, flexible cords on the foot pedals had the outer sheathing pulled back causing strain and abrasion to the inner conductors;    b) at the 570 grind table area, the flexible grounding cable had strain, being pulled where the cord was routed through a metal pipe in the floor; and    c) at the 545 PEM area, the HAEGER rivet machine operates at 480 volts and the foot pedal flexible cord sheathing was pulled back and ripped causing strain and abrasion to the exposed conductors.
