On or about December 12, 2013, on a 4:12 pitch roof, employees installing sheathing were not protected from a 22 feet exterior fall hazard by the use of a fall protection system. b. On or about December 12, 2013, on a 4:12 pitch roof, employees installing sheathing were not protected from a 9-feet 3-inches interior fall hazard by the use of a fall protection system.
