29 CFR 1926.454(a)(1): The employer failed to have each employee who performs work while on a scaffold trained regarding the nature of any electrical hazards, fall hazards and falling object hazards in the work area:.    JOBSITE:  The employees installing siding to the exterior of a club/leasing house and exposed to the hazard of falling had not received training and information to recognize the hazards associated with the type of scaffold being used.

JOBSITE:  The employee working from a Werner 16 ft aluminum extension ladder was not trained by a competent person in the proper procedures to minimize the hazards associated with ladders.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.    JOBSITE:  The employees, installing siding to the exterior of a club/leasing house, were not provided with a safe means of access while climbing up and down from the pump jack scaffold.

29 CFR 1926.451(g)(4)(v): When screens and mesh are used, they did not extend from the top edge of the guardrail system to the scaffold platform, and along the entire opening between the supports.:    JOBSITE:  The safety net/mesh, installed to protect employees from falling 12 ft 9 inches to the ground while the employees were installing siding to the exterior of a house from the platform of a pump jack scaffold, did not extend along the entire length of the opening between the supports.    The New Walk Home Improvement, LLC was previously cited for a violation of an equivalent occupational safety and health standard {29 CFR 1926.451(g)(4)(i)}, which was contained in OSHA inspection number 919044, citation number 01, item number 001 and was affirmed as a final order on 08/22/2013, with respect to a workplace located at 4 Cedar Ct, Hackettstown, NJ 07840.
