29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by rotating parts:        (a)  Employee(s) operating the Jet lathe along the north wall of the maintenance shop were exposed to struck by hazards due to no chuck guard being provided for a 14 inch chuck.  The Jet lathe operates between 80 - 1800 revolutions per minute.        (b)  Employee(s) operating the Old Milwaukee horizontal band saw (serial number:  YN0071) in the maintenance shop were exposed to struck by hazards due to no guard being provided for the unused section of the rotating blade.  The unused section of rotating blade was approximately 7-1/2 inches on the left side of the cutting area.  The horizontal band saw operates at 1720 revolutions per minute.       (c)  Employee(s) operating the Old Milwaukee horizontal band saw (serial number:  YN0071) in the maintenance shop were exposed to struck by hazards due to no guarding being provided for the unused section of the rotating blade.  The unused section of the rotating blade was approximately 5 inches on the right side of the cutting area.  The horizontal band saw operates at 1720 revolutions per  minute.

29 CFR 1910.219(b)(1): Flywheel(s) with parts seven feet or less above floor(s) or platform(s) were not guarded in accordance with the requirements specified in 29 CFR 1910.219(b)(1)(i) through (b)(1)(iv)(a):   Employee(s) operating the Bostich manual stitch 1 & 2 in the side tear area were exposed to caught in between hazards due to the openings around the flywheel not being guarded to prevent inadvertent contact with a rotating part.  The opening around the top of the flywheel was approximately 1-1/2 inches and around the bottom of the flywheel the opening was approximately 4 inches.  The speed of the flywheel was approximately 700 revolutions per minute using a 1/3 horsepower motor.

29 CFR 1910.219(h)(1): Projecting key(s), setscrew(s), or other projections in revolving part(s) were not removed, made flush, or guarded by metal cover(s):     Employee(s) operating the Baumfelder 714 in the small quantity area were exposed to struck by hazards due to no guard being provided for the rotating drive wheel spindle on the left side of the machine.  The drive wheel spindle was extending out approximately 3/4" from the machine and rotating up to 1248 revolutions per minute.

29 CFR 1910.303(g)(2)(i): Live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):   (a) Employee(s) operating the label inserter under the mezzanine were exposed to electrical hazards due to the control panel not being fully enclosed to prevent inadvertent contact with live parts that were rated at 208 volts.   (b)  Employee(s) working in the side tear area were exposed to electrical hazards due to the 3" x 4" side openings on the Folio #1 electrical box not being enclosed to prevent inadvertent contact with live parts that were rated at 120 volts.
