29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: (a) A metal four-plex box containing knock outs was being used as part of an extension cord instead of the box being fixed in a stationary application. Employees were subject to electrical shock hazards. Abatement documentation has been provided for this violation.

29 CFR 1910.303(b)(7)(iv): There were damaged parts that may adversely affect safe operation or mechanical strength of the equipment, such as parts that are broken, bent, cut, or deteriorated by corrosion, chemcial action or overheating. (a) The face plate of the electrical control pedant which raised and lowered the hydro table was damaged and held in place with duct tape which allowed access to energized parts. Abatement documentation has been provided for this violation.
