On or about December 12, 2013, one employee was exposed to laceration and amputation hazards when using a Makita circular saw, model #5007 F with no serial number, having the saw blade guard shimmed in the open position, while cutting the staircase stringers.

On or about December 12, 2013, employees were exposed to fall greater than 6 feet in height to the concrete floor below, while installing the pre-fabricated roof trusses over the foyer/living room area of the residential home. b. On or about December 12, 2013, employee were exposed to fall hazards greater than 6 feet in height to the concrete floor below, while installing the pre-fabricated roof trusses over the garage area of the residential home.
