29 CFR 1926.450(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports: On or about December 12, 2013, employee exposed to falls hazards of up to 13 feet 8 inches to the ground level below, when working from a welded frame scaffold which had 3 of a possible 4 plankings, while employee was sitting on the top platform of the scaffold cleaning the bricks.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section. On or about December 12, 2013, employee was exposed to fall hazards greater than 10 feet in height to the ground level below, when working from the scaffold which did not have guard railing at the ends of the scaffold platform, while cleaning the brick.
