(b) Warehouse & Cooler: On or about December 10, 2013, and at times prior, employees were exposed to a struck by hazards while working in and around storage racks that were not securely fastened to the floor. (d) Warehouse & Cooler: On or about December 10, 2013, and at times prior, employees were exposed to a struck by hazards while working in and around storage racks that were not properly guarded against minor impacts.
