29 CFR 1903.19(d)(1): The employer did not submit to the Agency, along with the information on abatement certification required by paragraph (c)(3) of this section, documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required:  (a)    The employer did not submit an abatement certification that included documentation demonstrating that abatement was completed for citation 1 for 1a) the abrasive bench grinder in the shop area having a left guard; 1b) the work rest being one inch from the abrasive wheel on the bench grinder located in the shop area; 1c) a missing tongue guard on the bench grinder located in the shop area; 2) mechanics serving single-piece rim wheels not being trained and instructed on the correct procedures including the safe operating procedures in paragraph g; 3) the training program did not include the applicable charts and rim manuals and the content of this standard; 4) an evaluation of each diesel mechanic's ability to service single-piece rim wheels; citation 2 for 1) a written certification that the workplace hazards has been assessed for the proper PPE; 2) providing at no cost to the mechanics fire resistant protective jackets; 3) ensuring that the 20 Ton Air/Hydraulic jacks were inspection at least once every six months; and 4) developing and implementing a hazard communication program.

29 CFR 1910.215(a)(4): Work rests. On offhand grinding machines, work rests shall be used to support the work. They shall be of rigid construction and designed to be adjustable to compensate for wheel wear. Work rests shall be kept adjusted closely to the wheel with a maximum opening of one-eighth inch to prevent the work from being jammed between the wheel and the rest, which may cause wheel breakage. The work rest shall be securely clamped after each adjustment. The adjustment shall not be made with the wheel in motion.  In the Shop Area, on the bench abrasive grinder, an employee was exposed to struck-by hazard using the grinder with the work rests being located one inch from the abrasive wheels.

29 CFR 1910.215(b)(9): Exposure adjustment. Safety guards of the types described in Subparagraphs (3) and (4) of this paragraph, where the operator stands in front of the opening, shall be constructed so that the peripheral protecting member can be adjusted to the constantly decreasing diameter of the wheel. The maximum angular exposure above the horizontal plane of the wheel spindle as specified in paragraphs (b)(3) and (4) of this section shall never be exceeded, and the distance between the wheel periphery and the adjustable tongue or the end of the peripheral member at the top shall never exceed one-fourth inch. (See Figures O-18, O-19, O-20, O-21, O-22, and O-23.)  In the Shop Area, on the bench abrasive grinder, an employee was exposed to a struck-by hazard while using the grinder with a missing tongue guard on the left eight inch abrasive wheel.

29 CFR 1910.177(c)(1)(i): The employer shall assure that no employee services any rim wheel unless the employee has been trained and instructed in correct procedures of servicing the type of wheel being serviced, and in the safe operating procedures described in paragraphs (f) and (g) of this section.  Diesel mechanics who service single-piece rim wheels had not been trained and instructed on the correct procedures for the type of wheel being serviced and in safe operating procedures in paragraph (g).
