29 CFR 1910.305(b)(2)(i): Pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose: On or about December 12, 2013, at the above addressed jobsite, two wall mounted electrical boxes located in the ammonia compressor room, were missing the cover plates. This exposed the employee to electrical hazards from the energized wires inside the box.
